As a courtesy to you we will submit your insurance claim to your carrier and answer any questions you may have regarding coverage.
Late/Cancellation Policy
Please arrive 5 min prior to your scheduled appointment to update insurance or personal information that may have changed since your last visit. We try to accommodate every patient and to make it fair; if you are more then 15 minutes late we may need to reschedule your appointment. We understand emergencies are uncontrollable especially when dealing with children; therefore the first cancellation that was not made within 24 hours of the appointment or a no show appointment will be a no charge courtesy, the second cancellation or no show appointments will be assessed $10 per child, the third $25. After the third cancellation per year we will then determine your status with the practice.
Financial
Before treatment we will work with your insurance to provide a cost estimate treatment plan for your child. Please understand this is an estimate only and not a guarantee of benefits. Payment is due at the time of service. After claim submission you are responsible for any remaining balance due and have 30 days from the receipt of treatment to pay the amount in full.
